I hereby give Micah Lynn Photography permission to use images from this session for possible forms of advertisement to promote the photographers name or business.  I confirm that I have legal authority to grant these permissions for myself and the included minor children.

I agree to show up for ALL scheduled appointments.  Failure to show up as agreed, or repetitive rescheduling (defined as rescheduling an appointment more than 2 times) will result in an additional charge of session fee amount that must be paid before the next appointment for either a session or an ordering appointment.  24 hour notice MUST be given if you aren't able to make a scheduled appointment.  Anything less than 24 hours will be considered a no show, in that event you agree to forfeit any deposit paid and understand an additional payment must be paid in order to reschedule.
